PATIENT INFORMATION NOTICE

This information notice has been prepared by Dentadent Oral and Dental Health Center, in its capacity as data controller, within the scope of Article 10 of the Law No. 6698 on the Protection of Personal Data and the Communiqué on the Procedures and Principles to Be Followed in Fulfilling the Obligation to Inform.

1. Identity of the Data Controller

Pursuant to the Law No. 6698 on the Protection of Personal Data (“Law No. 6698”), your personal data may be processed by Dentadent Oral and Dental Health Center (the “Clinic”), acting in its capacity as data controller, within the scope explained below.

2. Purposes of Processing Personal Data

Your personal data are processed in accordance with the Law No. 6698 on the Protection of Personal Data and secondary regulations within the framework of the purposes and legal grounds specified below.

The identity information, contact details, physical premises security information, health data, and legal transaction information of our patients shall be processed for the following purposes:

ensuring that activities are carried out in compliance with the applicable legislation,
execution of finance and accounting operations,
conducting and auditing business activities and ensuring business continuity through advertising and marketing activities,
execution of treatment processes,
providing information to authorized persons, institutions, and organizations,
ensuring physical premises security,
resolution of legal disputes,
conducting storage and archiving activities,
execution of processes related to customer loyalty regarding company/products/services,
conducting communication activities.

3. Method and Legal Basis of Collecting Personal Data

Our Clinic collects your personal data manually through the completion of forms submitted to you.

Your personal data shall be processed based on the legal grounds set forth in Articles 5/2-ç and 5/2-f of the Law No. 6698 on the Protection of Personal Data, in line with the purposes and legal grounds specified above, and shall be processed for the legally prescribed retention periods and stored for a limited duration.

4. Transfer of Personal Data

Within the framework of the purposes and legal grounds specified above, our Clinic may share your personal data processed with third-party business partners with whom it carries out its activities and/or with whom it is required to share data for the performance of services, or with legally authorized public and private institutions upon request, pursuant to Article 5/2-ç of Law No. 6698.

5. Rights of the Data Subject

Natural persons whose personal data are processed within our Clinic have the following rights pursuant to Article 11 of the Law:

To learn whether their personal data are processed,
To request information if their personal data have been processed,
To learn the purpose of processing and whether personal data are used in accordance with their purpose,
To know the third parties to whom personal data are transferred domestically or abroad,
To request the rectification of incomplete or inaccurate personal data and to request notification of such rectification to third parties to whom personal data have been transferred,
To request the deletion or destruction of personal data where the reasons requiring processing cease to exist, even though they have been processed in accordance with the KVKK and other relevant legislation, and to request notification of such action to third parties to whom personal data have been transferred,
To object to the occurrence of a result against the person by analyzing the processed data exclusively through automated systems,
To request compensation for damages in case of unlawful processing of personal data.

The data subject may submit requests regarding these rights in accordance with the Communiqué on the Procedures and Principles for Application to the Data Controller. In order to exercise your rights specified above, you may personally deliver your written request containing the necessary identification information or send it through a notary public.

The requests of the data subject shall be evaluated and concluded free of charge as soon as possible and no later than thirty (30) days. In the event that the evaluation and decision-making process requires an additional cost, the fee determined by the Personal Data Protection Board shall apply.
