Identity of the Data Controller<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Pursuant to the Law No. 6698 on the Protection of Personal Data (\u201cLaw No. 6698\u201d), your personal data may be processed by Dentadent Oral and Dental Health Center (the \u201cClinic\u201d), acting in its capacity as data controller, within the scope explained below.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Website: <a href=\"https:\/\/www.dentadent.com.tr\/\">https:\/\/www.dentadent.com.tr\/<\/a><\/li>\n\n\n\n<li>Telephone Number: +90 212 660 4400<\/li>\n\n\n\n<li>Email Address: info@dentadent.com.tr<\/li>\n\n\n\n<li>Address: Zuhurat Baba Neighborhood, Y\u00fcce Tarla Street, Tamburac\u0131 Osman Street No: 8, Bak\u0131rk\u00f6y, Istanbul<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Personal Data Processed in Relation to Employees<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Within our Clinic, personal data relating to employees may be processed in line with the purposes and processing conditions specified in this notice. The personal data subject to processing are as follows:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>a. Identity Data: Name, surname, mother\u2019s and father\u2019s name, date of birth, Turkish ID number, gender, marital status, ID card series\/serial number, nationality information, passport information, duty and title information, information on family members\/relatives.<\/li>\n\n\n\n<li>b. Contact Data: Telephone number, email address, address information, internal communication details within the Clinic (Clinic telephone number, extension number, corporate email address, registered email address).<\/li>\n\n\n\n<li>c. Financial Data: Bank IBAN number, payrolls, file and debt information related to enforcement proceedings.<\/li>\n\n\n\n<li>d. Professional Experience Data: Education status, in-service training information, certificate and diploma information, language skills, courses attended, education and competencies, work experience, transcript information.<\/li>\n\n\n\n<li>e. Visual and Audio Data: Photograph of the natural person, camera recordings.<\/li>\n\n\n\n<li>f. Personnel Data: Payroll information, disciplinary investigation, employment entry-exit records, CV information, performance evaluation reports, insurance information, criminal record, leave start and return dates, military service status information, department and unit.<\/li>\n\n\n\n<li>g. Health Data: Medical report, blood type, personal health information, health and maternity leave documents, information on disability status, information on devices and prostheses used.<\/li>\n\n\n\n<li>h. Other Data: IP address information on corporate computers, website login-logout records on corporate computers, vehicle license plate, accommodation and travel information, handwriting and signature, request\/complaint information.<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\">2. Purposes of Processing Personal Data<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Within the scope of Article 10 of the KVKK and Article 5 of the Communiqu\u00e9, and in compliance with the processing conditions set forth in Article 4 of the KVKK, employees\u2019 personal data may be processed for the following purposes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Fulfillment of service obligations under the service contract,<\/li>\n\n\n\n<li>Fulfillment of employer responsibilities,<\/li>\n\n\n\n<li>Ensuring occupational safety; management, supervision, and performance of work,<\/li>\n\n\n\n<li>Receiving and evaluating suggestions aimed at improving business processes,<\/li>\n\n\n\n<li>Providing information regarding changes that may occur in our service terms,<\/li>\n\n\n\n<li>Resolving employees\u2019 complaints and processing data access or rectification requests,<\/li>\n\n\n\n<li>Conducting work permit and residence permit procedures for foreign personnel,<\/li>\n\n\n\n<li>Preparation of all records and documents that will serve as a basis for processing in electronic (internet\/mobile, etc.) or physical environments,<\/li>\n\n\n\n<li>Conducting contract processes within the scope of the Labor Law and all other applicable legislation,<\/li>\n\n\n\n<li>Providing information to public officials upon request and as required by legislation regarding matters related to public security,<\/li>\n\n\n\n<li>Fulfillment of legal obligations and exercise of rights arising from applicable legislation,<\/li>\n\n\n\n<li>Fulfillment of legal obligations in the event that the relevant authority requests information within the scope of judicial and administrative investigations and it is mandatory to respond,<\/li>\n\n\n\n<li>Conducting application processes of employee candidates,<\/li>\n\n\n\n<li>Conducting fringe benefits and employee benefits processes,<\/li>\n\n\n\n<li>Fulfillment of obligations arising from employment contracts and legislation for employees,<\/li>\n\n\n\n<li>Conducting training activities aimed at employee satisfaction,<\/li>\n\n\n\n<li>Conducting activities for providing training to employees,<\/li>\n\n\n\n<li>Execution of business activities,<\/li>\n\n\n\n<li>Conducting occupational health and safety activities,<\/li>\n\n\n\n<li>Execution of assignment processes,<\/li>\n\n\n\n<li>Conducting internal audit activities,<\/li>\n\n\n\n<li>Conducting performance evaluation processes,<\/li>\n\n\n\n<li>Providing information to authorized persons, institutions and organizations,<\/li>\n\n\n\n<li>Conducting emergency management processes,<\/li>\n\n\n\n<li>Conducting communication activities,<\/li>\n\n\n\n<li>Execution of accounting and finance operations,<\/li>\n\n\n\n<li>Organization and event management,<\/li>\n\n\n\n<li>Execution of information security processes,<\/li>\n\n\n\n<li>Conducting storage and archiving activities,<\/li>\n\n\n\n<li>Ensuring physical premises security.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">3. Method and Legal Basis of Collecting Personal Data<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Personal data may be obtained directly from the data subject, from third parties, and from legal authorities during the establishment of a legal relationship. Within this scope, personal data may be collected verbally, in writing, or electronically via tools such as email and application forms and through written or verbal communication channels. In order for employers to fully and accurately fulfill their obligations arising from employment contracts and laws\u2014particularly under labor and social security legislation\u2014personal data may be collected.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">4. Transfer of Personal Data<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">For your security and in order for our Clinic to fulfill its obligations under the laws, your personal data may be shared, to the extent permitted and required by the relevant legislation\u2014 including but not limited to the Labor Law, the Occupational Health and Safety Law, the Social Insurances and General Health Insurance Law, the Law on the Regulation of Publications on the Internet and Combating Crimes Committed Through Such Publications, the Turkish Commercial Code, the Law No. 6698 on the Protection of Personal Data, and the Identity Notification Law\u2014with relevant institutions or organizations and public legal entities such as the Personal Data Protection Authority, the Ministry of Treasury and Finance, the Ministry of Customs and Trade, the Ministry of Labor and Social Security, the Turkish Employment Agency (\u0130\u015eKUR), and the Information and Communication Technologies Authority. For example, employees\u2019 personal data are shared with the Social Security Institution in order to enable the payment of employee and employer premiums.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In addition, we may transfer your personal data for the following purposes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>For the fulfillment of the employment contract and the realization of the required purpose, in particular:\n<ul class=\"wp-block-list\">\n<li>We may process your data in our accounting software in order to carry out payroll operations and update relevant data. Such data are stored within the application\u2019s own data recording environment.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>In order to fulfill requirements under the Labor Law, the Occupational Health and Safety Law, the Social Security Law and relevant legislation, as well as other laws and regulations, in particular:\n<ul class=\"wp-block-list\">\n<li>Within the scope of banking legislation, we may share data with banks regarding individual pension fund deductions (BES) and salary account\/salary payments.<\/li>\n\n\n\n<li>We may share your health data with our workplace physician in order to enable medical treatment and health check-ups.<\/li>\n\n\n\n<li>We may transfer payroll information to Clinic auditors so that they can carry out audit activities.<\/li>\n\n\n\n<li>We may transfer your data to contracted supplier companies via salary and payroll application software in order to perform personnel calculations.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>In order to ensure security within the Clinic, in particular:\n<ul class=\"wp-block-list\">\n<li>For workplace security purposes, we may transfer data to the building management of the workplace to monitor entry and exit.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Due to our legal obligations, in particular:\n<ul class=\"wp-block-list\">\n<li>In order to exercise our right of defense, we may share your data with our lawyers and, provided that it is in accordance with law and procedure, with relevant institutions within the scope of our obligation to comply with legal requests such as court orders or requests for evidence.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>For the administration of the Clinic, the conduct of business, and the implementation of Clinic policies, in particular:\n<ul class=\"wp-block-list\">\n<li>We may transfer your personal data to public institutions to which we are affiliated or with which we cooperate, in order to ensure internal operations and to regulate working processes.<\/li>\n\n\n\n<li>Where necessary, your personal data may also be transferred to the service provider company for purposes such as transportation, vehicle supply, business card printing, and parking registration.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">5. Rights of the Data Subject<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Data subjects have the following rights pursuant to Article 11 of the Law:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>To learn whether their personal data are processed,<\/li>\n\n\n\n<li>To request information if their personal data have been processed,<\/li>\n\n\n\n<li>To learn the purpose of processing and whether personal data are used in accordance with their purpose,<\/li>\n\n\n\n<li>To know the third parties to whom personal data are transferred domestically or abroad,<\/li>\n\n\n\n<li>To request the rectification of incomplete or inaccurate personal data and to request notification of such rectification to third parties to whom personal data have been transferred,<\/li>\n\n\n\n<li>To request the deletion or destruction of personal data where the reasons requiring processing cease to exist, even though they have been processed in accordance with the KVKK and other relevant legislation, and to request notification of such action to third parties to whom personal data have been transferred,<\/li>\n\n\n\n<li>To object to the occurrence of a result against the person by analyzing the processed data exclusively through automated systems,<\/li>\n\n\n\n<li>To request compensation for damages in case of unlawful processing of personal data.<\/li>\n\n\n\n<li>The data subject may submit requests regarding these rights in accordance with the Communiqu\u00e9 on the Procedures and Principles for Application to the Data Controller. In order to exercise the above-mentioned rights, you may submit your petition including the necessary information to verify your identity in person, via notary public, or through other methods determined by the Personal Data Protection Board.<\/li>\n\n\n\n<li>The requests of the data subject shall be evaluated and concluded free of charge as soon as possible and no later than thirty (30) days. In the event that the evaluation and decision-making process requires an additional cost, the fee determined by the Personal Data Protection Board shall
